To be appointed as a notary public, you must be a practicing lawyer or paralegal in Ontario,
licensed by and in good-standing with the Law Society of Ontario.
This portal is ONLY for requesting new notary public appointment certificates.

After you submit your appointment application, you will receive an email confirmation with a service request number.
To complete your application, please mail or bring your payment in-person to Official Documents Services.
Official Documents Services will only process your application once payment is received.

The ministry’s collection and use of your personal information is for the purposes of administering the service such as initiating the service request, to send confirmation emails, to address any issues with service request or payment and to mail the certificate of appointment.
